Field Trip Waiver of Liability and Agreement

By completing this registration form, I indicate that I understand and agree to the above stipulations and to the following release of liability:

I wish to participate in a field trip sponsored by the Wisconsin Society of Ornithology (WSO).  I understand that this is a field trip that may require rigorous physical activity, attention to traffic both as a driver and as a pedestrian, and that it may be potentially hazardous. I attest and verify that I am physically fit and sufficiently trained for the field trip and that, if appropriate, my physical fitness to participate in this event has been verified by a licensed medical doctor.

I expressly assume, for myself and any minors accompanying me, all known and unknown risks associated with the Field Trip, including but not limited to: loss of or damage to personal property; injury (including death); accidents; the effect of weather; terrain conditions that may vary widely, and may include uneven and/or slippery surfaces; animals; natural and manmade obstacles that may unexpectedly present themselves, and the possibility that we may be exposed to and be infected by COVID-19.

I understand that the WSO is not responsible for my safety or the safety of my belongings on this field trip. I agree to hold harmless WSO, its Board Members, volunteers, and staff from and against any liability or causes of action in connection with my participation in this event.

I further agree that I am at least 18 years of age and have the ability to enter into this agreement and electronically sign this release to that effect by completing the registration process.

Photo Release: I hereby grant and convey to WSO all rights, title and interest in any and all photographic images, video, and/or audio recordings made by or on behalf of WSO during my field trip on this date including but not limited to any royalties, proceeds, or other benefits derived from such photographs or recordings. I further agree that WSO may photograph and/or videotape me while I am participating in this field trip and that WSO retains the rights to use these visual images and recordings in any manner without compensation to or prior authorization by me.
